Release and Waiver of Liability

The physical waiver form will be made available during the event. Any individual participating in a sporting event is required to acknowledge and sign the waiver prior to their first game.
 

In consideration of being allowed to participate in the Green Bay Hmong New Year 26/27 Volleyball Tournament, I, the undersigned, acknowledge and agree to the following:
 

1. Assumption of Risk
   I understand that participation in volleyball and related activities involves physical exertion and carries the risk of injury, including but not limited to sprains, strains, fractures, or other serious injuries. I voluntarily assume all risks, known and unknown, associated with my participation.
    

2. Release of Liability
   I hereby release, discharge, and hold harmless Peb Yog Hmoob, the University of Wisconsin–Green Bay (UWGB), event organizers, volunteers, sponsors, and affiliates from any and all claims, demands, liabilities, or causes of action arising from injuries, accidents, or damages incurred during the tournament.
    

3. Medical Authorization
   In the event of an injury or medical emergency, I authorize tournament staff or volunteers to obtain necessary medical treatment on my behalf. I understand that I am solely responsible for any costs related to such care.
    

4. Acknowledgment of Rules
   I agree to follow all rules, instructions, and safety guidelines provided by tournament officials and understand that failure to do so may result in my removal from the event.
